Insurance Coverage

Is the Virtual Visit Covered?
Every insurance is different but we bill the following codes for the 2 visits
Initial Evaluation- 99204
Follow-up Visit- 99213
We are out of network but will submit the claims on your behalf for a $25 fee. If insurance covers the visit and or custom orthotics we will process a refund on your credit card.
Are custom orthotics covered?
With hundreds of insurances available for patients to choose we are unable to know your exact coverage but once seen will share how to assess coverage with your insurance. In general federal and state-sponsored plans ( Medicare and Medicaid ) do not cover custom orthotics.
The code we use for custom orthotics is L3000
However, to assess coverage most insurances will require a diagnosis which out provider will give you.
Do I need a video to this appointment?
Yes, we will not conduct the appointment without viewing you, your feet, your stance and biomechanics of how you walk.
Can't i get custom orthotics from a local physician?
Yes, you can!
